Visual Form Editor licensing

All licensing options described below are for SINGLE-USER licenses. Please note that all licensing is also based on individuals using the software, NOT number of machines where the software is installed or number of concurrent users. There are several types of licensing which can be purchased for Visual Form Editor.

Clinical License: Intended for those who use Visual Form Editor to build forms for use in the clinic / practice with which they are employed. Forms created for this purpose can be SHARED free of charge with other clinics / practices. Forms can not be developed for a fee or sold for a fee. This license can not be used for consulting. Forms which are provided as part of a package of services for which a fee is charged (e.g. ASP services) are considered to be sold for a fee. Pricing for a single user clinical license is $1,500 which includes any upgrades for three months. An optional annual subscription which includes all upgrades is available for this license type at $500/year. The first payment for the subscription would be due one year from the initial purchase date, but enrollment should be done at the time of initially purchasing the license or within the three month period thereafter; otherwise there may be an additional charge for new releases after that period.

Consulting License: Intended for those who use Visual Form Editor to build forms as a consultant. This license requires that any clinics that are provided consulting services own at least one clinical license for Visual Form Editor. This license does NOT allow sale of forms that have been developed, but only allows charging a reasonable hourly rate for hours worked for the customer. Forms which are provided as part of a package of services for which a fee is charged (e.g. ASP services) are considered to be sold for a fee. Therefore, it also does NOT allow bundling of forms created with other services provided for a global fee. It does NOT allow consulting for clinics that do not own a license for Visual Form Editor. Forms created with this license can be SHARED free of charge with other clinics / practices through a web site where they are generally available (e.g. User Group Forum, NOT just available to customers of the consultant). Pricing for a single user consulting license is $2,500 which includes any upgrades for three months. An optional annual subscription which includes all upgrades is available for this license type at $750/year. The first payment for the subscription would be due one year from the initial purchase date, but enrollment should be done at the time of initially purchasing the license or within the three month period thereafter; otherwise there may be an additional charge for new releases after that period.

Enterprise License: Intended for those who use Visual Form Editor to build forms within an ASP or similar environment. This license allows development of content for clients that are hosted on a shared database. It allows for development of content for clinics that are hosted on the servers managed by the organization. This license does NOT allow sale of forms that have been developed. Forms which are provided as part of a package of services for which a fee is charged (other than the hosting of the EMR database as noted above) are considered to be sold for a fee. Therefore, it also does NOT allow bundling of forms created with other services provided for a global fee. It does NOT allow consulting for clinics whose database is not hosted by the license holder. Forms created with this license can be SHARED free of charge with other clinics / practices through a web site where they are generally available (e.g. User Group Forum, NOT just available to customers of the consultant). Standard pricing for a single user enterprise license is $2,500 with a MANDATORY ongoing subscription fee of $1,500/license/year starting one year after initial purchase. However, pricing may vary depending on exact use. The annual subscription fee includes all upgrades for new releases of Visual Form Editor.

Commercial License: This license can be used for the purpose of creating forms which may be sold to another organization or practice. It can also be used to create forms as part of a consulting arrangement and does NOT require that clients own a license for Visual Form Editor. This can be used by organizations that are not affiliated with a clinic, but consult for clinics or build forms under a contract. For further pricing information contact .
